After being arrested under the suspicion of a DUI, the MVD requires a DUI Screening (ARS 28-1385) to meet state requirements. The DUI Screening is an assessment tool used to determine hours of education, treatment or both you will be required to complete.
Arizona DUI Services will conduct your screening via therapy notes which connects you to your Screener in a video conference room.
To complete your DUI Screening, you will need an internet connection and a smart device/computer with a camera function. You will also need to provide proof that you need a DUI Screening by submitting a copy of your citation, court documents or an extended certified MVR. Lastly, a copy of a state issued ID is required. Without the photo ID and proof that you need a screening the reporting to the courts and/or MVD will be delayed until both are provided.
Level 1 and Level 2 DUI’s require 16 hours of Education. Our online 16 Hour DUI Education course meets the Arizona Department of Health Services and MVD’s requirements. Our online option allows you to complete your court ordered Education at your convenience with same day or next day appointment.
To complete your 16 hour DUI Education course online, you will need an internet connection and a smart device/computer. You will also need to provide proof that you need the 16 hour course by submitting a copy of your screening note (if another facility completed your screening), MVD correspondence; you are eligible for the interlock deferral program or court documents.
Lastly, a copy of a state issued ID is required. Without the photo ID and proof you need to complete the Education, reporting to the courts and/or MVD will be delayed until Arizona DUI Services have both items.
Driving is a privilege! Follow these steps as directed!
1. Pull your extended- certified MVR at the MVD or at azmvdnow.gov and email it in PDF format, ALONG with your completed DUI treatment certificates and your picture ID, to State Approved Evaluator- Evelyn Tovar, at eve.perspectivestherapy.com.
2. Upon MVR and DUI Treatment approval, Evelyn will email you a questionnaire form. Please reply to the email and respond to each question. Once the questionnaire is complete in the email and you are approved to move forward, Evelyn will send you a link to make your $120 payment. once you have paid your $120, please email evelyn this payment update.
If you have THREE or more DUI’s and are not in remission, please know that Evelyn cannot proceed with you and this Revocation Evaluation process.
3. Once you have been approved to move forward and have submitted your $120 payment, you will be sent a link to schedule your FINAL ONLINE appointment for your Substance Abuse Evaluation with Evelyn’s assistant, Carolina or Scott, along WITH THE revocation application. You must complete the top of the application and send it back to Evelyn in PDF, either through email or by mail.
4. Once your online Evaluation is complete, Evelyn will submit your application to the Driver Improvement team at ADOT MVD. You will then receive a copy by email as well.
You must follow these steps and be eligible for this process for the state-approved evaluator to sign your application and submit it to driver improvement.

Document Submission Deadline
You have a maximum of 30 days to submit the required documents after requesting the DUI Alcohol and Drug Screening service. If you exceed this timeframe, we will be unable to process refunds. For any questions, feel free to contact us.
